Birth Records

You can request and purchase a certified copy of your child’s birth certificate from the local registrar’s office, located in the city where the birth occurred, or from the Texas Vital Statistic office, located in Austin, Texas, approximately two to three weeks after the birth of your child.

Medical Records FAQs

Can I request my medical records online?

For the convenience of our patients and/or their legal guardians, you now have the option to request and receive copies of your medical records online more securely with a tracking number to track the progress. The online option provides faster turnaround time for your request, as it eliminates the delays caused by traditional mail.

Request records

To request your medical records online, you must have the following:

  • A smartphone with a camera and texting capability to submit the picture of a government-issued ID and a picture of yourself.
  • Legal guardians or healthcare proxies will need to upload a PDF or TIF copy of court-appointed guardianship papers, power of attorney or other appropriate legal documents that can establish proof of guardianship.

THE FOLLOWING WILL NOT BE RELEASED UTILIZING THIS METHOD: MENTAL HEALTH RECORDS, DECEASED PATIENT RECORDS AND RECORDS FOR PATIENTS BETWEEN THE AGES OF 12 and 17.

Both Verisma and Palo Pinto General Hospital are dedicated to the highest standards of privacy and security of your protected health information and complies with state and federal regulations.

Can I receive any medical records information verbally?

The only information the HIM Department releases verbally is the patient’s date of admission, and you will need to provide the last four numbers of the patient’s SSN as well as their date of birth.

When will I receive my baby's social security card?

If you answered “Yes” to the question “Apply for baby’s social security number?” the birth information will be forwarded to the Social Security Administration as soon as the Texas Vital Statistic office receives the data from the hospital. The Social Security Administration then requires two to three weeks to process the information. A Social Security card will be mailed to the mother’s mailing address, as provided in this worksheet. The entire process usually takes four to six weeks to complete.

When will I receive my baby's Medicaid number?

If you provided an answer for the question “Mother Medicaid Name?” and “Mother Medicaid Number?” the birth information will be forwarded to the Medicaid office as soon as the Texas Vital Statistic office receives the data from the hospital. Medicaid then requires two to three weeks to process the information. An Infant Medicaid card will be mailed to the mother’s mailing address, as provided in this worksheet. The entire process usually takes four to six weeks to complete.

Palo Pinto General Hospital uses text messaging to help keep you informed about your healthcare. Depending on your communication preferences, you may receive text messages regarding:

Providing your mobile phone number allows us to receive these communications. Participation is voluntary and is not required to receive medical care.
